CAP Inspection Checklist: What to Prepare Before Your Next Survey
CAP inspections are rarely failed because of one dramatic problem. They're failed — or rather, deficiencies are cited — because of small, boring gaps: a signature that's missing, a review date that slipped, a competency assessment that's six months overdue. This checklist is organized the way most labs actually get surprised: by category, with a rough timeline for when to start looking.
A rough prep timeline
Full document control audit
Check every SOP's review date against your review-frequency policy. Anything overdue needs to be reviewed and re-signed now, not the week of inspection.
Personnel & competency sweep
Every analyst testing patient samples needs current competency assessment covering all six required CLIA elements, for every test they perform.
QC and PT record check
Pull the last 12 months of QC data and look for gaps, unresolved out-of-range results, or missing corrective action documentation. Confirm PT enrollment is current and all results were reviewed and signed.
Validation & equipment file dry run
Pick two or three instruments at random and time yourself pulling the complete validation package for each. If it takes more than five minutes, it's not organized well enough.

📋 Document Control & SOPs
- Every active SOP has a current effective date and a review date within policy (commonly every 2 years, but check your own document control procedure).
- Superseded versions are archived, not deleted, and are clearly marked as superseded.
- Staff acknowledgment ("read and sign") records exist for every current SOP, for every relevant employee — including anyone who joined or changed departments recently.
- New or revised procedures were approved by the laboratory director before staff started using them.
👩🔬 Personnel & Competency
- Personnel files include current licenses/certifications where required by your state.
- Competency assessment on file for every test each analyst performs, covering all required elements, within the required frequency (typically semi-annually in the first year, then annually).
- Documented corrective action for any competency gap identified.
🧪 Quality Control & Proficiency Testing
- QC run at the required frequency for every test, with documented review — not just run, but reviewed and signed.
- Every out-of-range QC result has a documented investigation and corrective action, not just a re-run.
- PT enrollment covers every regulated analyte you perform, results were submitted on time, and unsuccessful PT events have a documented root-cause investigation.
✅ Method Validation & Verification
- A complete study on file for every test system — new, modified, or annually re-verified — with dated laboratory director review and approval.
- Studies are organized per instrument in a way that can be retrieved quickly (a master validation package, not scattered folders).
- Acceptance criteria are documented and were met — or, if not met, there's a documented decision and rationale from the director.
🔧 Equipment & Maintenance
- Maintenance logs current for every instrument, at the manufacturer-specified frequency, with no unexplained gaps.
- Out-of-range maintenance readings have documented corrective action.
- Instruments taken temporarily out of service have that clearly logged, so gaps don't read as missed maintenance.
🛠️ Quality Management (CAPA & Incident Records)
- Open CAPA cases have documented root-cause analysis and a plan with a timeline.
- Closed CAPA cases include effectiveness verification — proof the fix actually worked, not just that an action was taken.
- Prior inspection findings (yours or a related department's) have been addressed and closed.
